Well over 500 views and no right answers, so here are the answers.
1. Dr. John D. Brock, D.O. of Kansas City, Mo. Flew every day for ten full years commencing Nov. 15, 1929 and ending Nov. 15, 1939.
2. Initially with his new Waco 10, but also flew a Waco ATO Taperwing after selling his Waco 10. He subsequently purchased a Waco "Cabin" and then a Monocoupe 90A which helped in inclement weather flights.
3. The flights were in the Kansas City area of at least 15 minutes duration except one flight that just after takeoff flew into a sudden snowstorm with whiteout conditions and he promptly landed straight ahead safely. His final flight on Nov. 15, 1939 had an offered, arranged escort of three US Navy Curtiss O2C Helldivers from Kansas City to St. Louis, Mo. Lambert Field. All flights were witnessed and as his national fame grew there were no shortage of onlookers wanting to sign his logbook.

Dr. Brock was a Kansas City Optometrist and sportsman pilot who owned the Specialty Optical Company in Kansas City. He learned to fly in a 1918 Standard biplane powered by an OX-5 in his youth and tried to fly every day. He purchased a Waco 7, but in the Spring of 1928 he bought a Waco 10. He spent much of his time in the air, as he loved to fly so much.
He flew at least a little every day for a year and then set upon attempting an official witnessed record of flying each and every day for a year starting on November 15, 1929. At the end of one year of flying 365 consecutive he was feted locally. He was feted again after completing 1,000 days of consecutive flights.
Year after year he continued flying daily using the various aircraft listed above. Obviously he had the time and the wealth to pursue such a goal. Some days he had to wait for a suitable weather window, but achieved his goal of flying every consecutive day for ten years. His exploit was heralded periodically during the years and and he became a household word. He was invited by the United States President Franklin Delano Roosevelt as his guest to the White house to receive congratulations after his record was achieved. Apparently, he feat has never been officially duplicated or topped.
