On Tuesday 7 October 2008 at about 5 pm, N454SA, a single engine 2007 Iniziative Industriale Italian SKY ARROW 600 SPORT aircraft crashed into the Pacific Ocean about 50 yards offshore near the Malibu pier in California. The Light Sport Aircraft with two on board had taken off from Santa Monica Airport. The occupants were rescued and were in critical condition, according to a Los Angeles County fire Captain. An FAA spokesman stated the aircraft was not in communication with air traffic controllers at the time of the crash.
